By submitting this application, I grant the Master Chimbala Foundation (MCF), its partners, supporters, agents, contractors, stakeholders, affiliates, sponsors, and collaborating institutions unrestricted permission to record, photograph, film, interview, reproduce, edit, publish, broadcast, distribute, archive, and otherwise use my name, image, likeness, voice, biography, personal story, statements, speeches, performances, photographs, audio recordings, video recordings, social media content, and any other materials generated or provided in connection with Thy Speech Master and related activities. I understand that such materials may be used without compensation for educational, awareness, advocacy, communication, branding, publicity, documentation, reporting, promotional, fundraising, media, digital, social media, publication, archival, and other lawful purposes in any format, platform, or medium, now known or developed in the future. I further acknowledge that the Master Chimbala Foundation and its authorised partners may use, adapt, edit, reproduce, translate, and distribute such content while preserving its intended meaning and purpose, and I waive any rights to inspect, approve, or claim ownership, royalties, or compensation arising from such use.

In the interest of maintaining the integrity, credibility, and public health objectives of this initiative, I hereby declare that I am not employed by, contracted to, sponsored by, funded by, acting on behalf of, or otherwise affiliated with any tobacco, nicotine, vaping, electronic cigarette, heated tobacco, or related industry entity. I further declare that, to the best of my knowledge, my participation in this programme does not present any conflict of interest arising from direct or indirect relationships with such industries. I undertake to disclose any actual, potential, or perceived conflict of interest that may arise during the course of the programme.
